If you have received a bill, but have questions, or believe it to be incorrect, please call the billing office at: 504-340-2337.

* For patients with no form of health care coverage, fees are assessed on a sliding scale based on the amount of household income and number of dependents. Patients who cannot pay the full amount for services provided, can elect to sign a promissory note, and pay over time. To establish credit, the patient will need to bring the following documents:

1. Picture Identification (state i.d., driver's license, school i.d., military i.d., etc.)

2. Statement of billing address (utility bill, vehicle registration, credit card bill, etc.)

3. Proof of income (employment check stubs for one month, unemployment statement, W-2, tax return, or letter of support from the person providing housing and expenses for the patient)
